Patient eMail Contact Form & Sicknote requests.
After many requests from our patients, as well as positive feedback in our practice surveys, we are pleased to introduce a trial of electronic communication with the surgery. This service will be reviewed over the coming months.
This is not to be used for clinical queries, especially anything of an urgent nature, and the Practice cannot take any responsibility for enquiries of this type - please ring to arrange appropriate assessment with a healthcare professional as usual. Any clinical queries will be ignored as these will only be seen by admin staff.
In order to use this service, (as well as receive a reply) we need to already have your email address on file, (having been completed on your repeat medication slip previously, or else confirmed in person in the Surgery with ID), in order to ensure confidentiality/security of your personal information etc.
Please remember that sending information through the website/email is not guaranteed as secure, so this is not a service for sending/discussing topics of a confidential or sensitive nature.

Available options:
1) Request Sicknote - Please provide dates required for sicknote, remembering that the first 7 calendar days of an absence should be covered by a 'self-certificate' to your employer. Please leave as much info as you can as to why you need the time off, have you seen a health profressional about this issue, what treatment are you having to try and get better, and if it is a repeat sicknote, what progress has there been since the last one has issued. Please remember that sicknotes are legal documets and the GP will not complete it if they do not have the neceesary amount of information.
2) Cancel an appointment - if you no longer need an appointment please complete this section, preferably giving us at least 24 hours notice, specifying the date/time and who the appointment was with, so we might be able to offer it to somebody else. We will send email confirmation that it has been cancelled. Please be aware currently you will still need to ring if you wish to re-book.
3) Any other queries - please include details in the free-text box, and remember clinical/medical queries will not be dealt with, please ring if this is the case.
4) Home BP Readings - If you need to submit blood pressure home readings, please select option 4, and then upload the document with the button provided.
5) Change Contact Details - you will need to provide both old and new details (address/telephone numbers) in order for us to change these. You will receive a confirmation email confirming changes.
