REGISTERING AS A PATIENT:
Acceptance by the Practice will depend on where you live (see diagram above indicating practice area).
Depending on your circumstances you will be accepted, or arrangements will be made for you to discuss your request for registration with one of the Partners.
Where patients are requesting to join the practice list, the practice does not discriminate on the grounds of:
Race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.
RIGHTS OF PATIENTS
As a patient of our practice you have the right to:
• Be treated as an individual, with courtesy, respect and dignity at all times
• Be offered a health check appointment on registration
• Be given the names of people involved in your care and/or treatment
• Be referred to a specialist acceptable to you when your doctor feels this course of action is necessary
• Expect a complaint to be handled in accordance with NHS guidelines
• Be given a copy of the practice leaflet on registration which details the services available to you.
We will try our best to meet any reasonable preference expressed to see a particular health care professional but need not do so if the preferred practitioner has reasonable grounds for refusing to provide services to the patient or does not routinely perform the service in question within the practice.
PATIENTS' RESPONSIBILITIES
• To treat the staff with courtesy and respect
• To ask for a home visit only when it’s necessary - when the patient is housebound or too ill to attend surgery
• To be punctual for appointments
• To inform the surgery if you are unable to keep an appointment
• To inform the surgery of any change of address or telephone number
• To be patient- delays are sometimes unavoidable
• To act in a responsible way whilst in the surgery and keep children under control
• To use the emergency out of hours only for problems that cannot wait until surgery reopens
• Be given the names of people involved in your care and/or treatment
• Be referred to a specialist acceptable to you when your doctor feels this course of action is necessary
• Be offered a health check
VIOLENT OR ABUSIVE PATIENTS
We operate a zero tolerance policy towards any patient who is threatening, violent or abusive to any of our staff or professionals. Such patients will be reported to the police and removed from the practice list. Patients will be informed in writing of the reasons for removal and the action they need to take to register with an alternative surgery.
