Patient eMail Contact Form & Fit Note requests
This service is not to be used for clinical queries, especially anything of an urgent nature, and the Practice cannot take any responsibility for enquiries of this type - please ring to arrange appropriate assessment with a healthcare professional as usual. Any clinical queries will be ignored as these will only be seen by admin staff.
In order to use this service, (as well as receive a reply) we need to already have your email address on file, (having been completed on your repeat medication slip previously, or else confirmed in person in the Surgery with ID), in order to ensure confidentiality/security of your personal information etc.
Please remember that sending information through the website/email is not guaranteed as secure, so this is not a service for sending/discussing topics of a confidential or sensitive nature.

1) Request Fit Note (sick note) - Please provide dates required for Fit Note, remembering that the first 7 calendar days of an absence should be covered by a 'self-certificate' to your employer. 'New/First' sicknotes will normally require a face to face or telephone appointment so we can better understand your situation. If completing the form below, please leave as much info as you can as to why you need the time off, have you seen a health professional about this issue, what treatment are you having to try and get better, and if it is a repeat sicknote - what progress has there been since the last one has issued. Please remember that sicknotes are legal documents and the GP will not complete one if they do not have the necessary information. For more info on Fit Notes, please click here.
2) Request/Amend/Cancel an appointment - if you are not able (or prefer not) to request an appointment via phone, then please complete this form, giving us any relevant info you think we may need, eg who you need to see: eg GP/nurse/pharmacist, what the appointment is for, and any days/times that are particularly (un)suitable. If cancelling an appointment, please give as much notice as you can, specifying the date/time and who the appointment was with, so we might be able to offer it to somebody else. We will send an email confirmation that it has been cancelled.
3) Any other queries - please include details in the free-text box, and remember clinical/medical queries will not be dealt with, please ring if this is the case.
4) Home BP Readings - If you need to submit blood pressure home readings, please select option 4, and then upload the document with the button provided.
5) Change Contact Details - you will need to provide both old and new details (address/telephone numbers) in order for us to change these. You will receive a confirmation email confirming changes.
